ey Facts - This is a travel insurance policy summary only and does not contain the full terms and conditions of the contract.Full terms and conditions can be found in the full policy wording ( copy available on request ) enclosed with your policy validation slip. Holiday Services Ltd is authorised by the Financial Services Authority ( FSA ), Firm Reference Number 309833. The Insurers - Fortis Insurance who are regulated and authorised by the Financial Servies Authority ( FSA ).  Single Trip and Annual Multi Trip Cover - The following details apply to the HX/ Fortis Scheme, Premier Cover Single Trip of up to 185 days duration, Annual Multi Trip of up to 31 or 60 days duration. The cancellation cover starts when cover is purchased, all other sections of cover start on the date of travel shown. When the validation slip is attached, the policy document becomes a contract between you and Fortis Insurance. It is the basis on which Fortis Insurance will settle any claim. You must have read the Medical Exclusions and Important Conditions before your cover can start. Significant Features and Benefits - The policy wording gives full details of the features and benefits of the cover provided under each section of the policy. The policy wording also has a table of benefits to give an overview of the sections of cover, the maximum sums insured and the amount of excess if it applies to those sections. The following is a summary of cover only -  

Principal Exclusions or Limitations. As with all insurances, there are exclusions and limitation to the cover provided. These are detailed in the policy wording after each section of cover or under general headings. You should check if your own requirements and circumstances are unaffected by them. If you have any queries in the first instance contact Holiday Services. Health - Health restrictions apply to some sections of this policy. Please refer to the  Medical Exclusions and Important Conditions below as you must keep to these conditions to have the full protection of your policy. If you do not we ( Fortis Insurance )  may cancel the policy or refuse to deal with your claim, or reduce the amount of any payment we make. If you do not declare a condition of yours or someone upon who successful travel may depend, a subsequent claim may not be met. See the Sections A, B and C below under the section How To Apply. If after purchasing these insurances, there is a change in health and you can no longer agree to the declaration, a subsequent claim may not be met. Your policy does not cover you if you have a terminal condition, are travelling against medical advice or to get medical treatment abroad.  

Dangerous Activities - your policy does not automatically cover you for all activities/sports. If you are going to take part any activity or sport that may be considered dangerous please contact Holiday Services. Personal Belongings, Money and Documents. Your Policy provides cover for loss, damage or theft of your personal belongings, money and documents. However, we ( Fortis Insurance)  may take an amount off for any claim settlement for age, wear and tear. The policy also contains maximum amounts that can be claimed for individual items, valuable items and cash. If you have property that exceeds the maximum amounts in the policy you may wish to insure such items under specific property insurance, such as household all risk policies. What happens if I take out cover and then change my mind? You can cancel these insurances within 14 days from the date of policy issue and certificate validation ( less a Holiday Services admin. fee of £2 per policy ), providing no claim has been made and your travel has not already started. No refund is available after the 14 day period. How to make a Claim - You should contact Fortis Insurance  0870 844 2807 quoting your validation slip number. You may have to provide documentation to support your claim. All details of how to make a claim will be included in the Policy Wording. Medical Emergencies Overseas - if you have a medical emergency on your trip please contact 0044 2380 644633. NB (a) if you need treatment in Australia or NZ, you must go to a state hospital and register with Medicare. This will entitle you to some free treatment. (b) If you are travelling in Europe, get a current valid E 111 Form or similar as required from your main post office and fill it in. This will entitle you to some free treatment.  What to do if you have any complaints - If you have any complaints about your policy in the first instance contact the broker, Holiday Services Ltd .You could then contact Customer Relations, Holiday Extras, Newingreen, Hythe, Kent CT21 4JF. 0870 844 2409. If your complaint cannot be resolved at this stage, contact the Head of Customer Care, Fortis Insurance Ltd, Fortis House, Tollgate, Eastleigh, SO53 3YA. If you have received a final response and are still not satisfied, you may refer your call to the Financial Ombudsman Service ( FOS ), Insurance Division, Financial Ombudsman Service, South Quay Plaza, 183 Marsh Wall, London E14 9SR. Full details will be included in your policy. Financial Compensation Scheme. In the event that Fortis Insurance Ltd is unable to meet it's liabilities, you may be entitled to compensation from the Financial Services Compensation Scheme. How to applyImportant - please refer to the information given below - A Pre-existing medical conditions B Waiting Lists and tests C Important Conditions. If nobody requiring cover needs to contact the Medical Screening Line you can print off the correct proposal form ( Single Trip or Annual Multi Trip Form )  and send this to Holiday Services enclosing the premium(s) as required.  If in doubt after reading these conditions  call the Medical Screening Line on 0845 458 0718 ( this number is charged at local call rates only ), Monday - Saturday 9am - 5pm.  Tell them  which policy you want to take out, Premier Cover Single Trip  or Worldwide Annual Multi Trip and that you have not yet taken out a policy but have a query regarding a pre-existing  medical condition or related matter after reading A, B and C below. NB The Medical Screening Line are only concerned with queries regarding  cover for medical related queries and they cannot issue your policy. You need to complete the proposal form  (which you can print out  on this site) and send this to Holiday Services together with the premiums due. The Medical Screening Line should be able  advise you if they can include cover for any declared condition(s). You may be accepted for cover at normal rates, you may have to  pay an extra premium to cover the declared condition(s) or there may be an increased excess on some sections of the policy.  Purchase of excess waiver does not remove any  special excess as advised by the Medical Screening Line.  Any decision from the Medical Screening Line  only lasts for a maximum of 14 days ( providing your medical condition does not change in this time ) so you will need to send your proposal form with premiums due to Holiday Services within 10 days. Doctors letters are not required  but you must not travel if your doctor advises you not to at any time before your trip.  Please enter any Medical reference Number given to you by the Medical Helpline  on the place  required on the proposal form. Please ensure that you have advised the Medical Helpline which type of policy you require eg Premier Cover, WW Annual Multi Trip or other. If you have to pay an additional premium please note that this is paid direct to the Medical Helpline  as soon as you receive your policy from Holiday Services or  have a policy number. You can call them and pay any extra amount due by credit/debit card. Do not add this extra amount to the normal premiums required by Holiday Services.    

Medical Exclusions and Important Conditions - please read. If you are not sure of your cover regarding any medical condition or medication after reading the following section, contact the Medical Screening Line and they should be able to check this for you.

Pre-existing medical conditions -

You will not be covered under the following sections in the circumstances shown below - Medical and other expenses ( including hospital benefit ), Loss of deposit or Cancellation, Curtailment ( cutting short your trip ), Personal accident and Personal liability.

We will not provide cover if you have -

* ever had any respiratory condition ( relating to lungs or breathing ), heart condition, stroke or cancer. for which you have ever received treatment ( treatment includes surgery, tests or investigations by a doctor, consultant or specialist, and prescribed drugs or medication ) or -

* had any medical condition within the last 12 months for which you have received surgery, in-patient treatment or investigations in a hospital or clinic, or for which you are taking prescribed medication.

However, you can call The Medical Screening Line on 0845 458 0718 and they may agree to provide cover, but they must confirm this to you in writing.

B Waiting lists and tests.

If you are on a waiting list for, or know that you need surgery, in-patient treatment or tests at a hospital, clinic or nursing home, we will not provide cover under the following sections of the policy for any claims arising from that need - Medical and other expenses ( including Hospital Benefit ), Loss of deposit or Cancellation, Curtailment, Personal accident and Personal Liability. ( NB if you are already  on a transplant list and have to cancel your trip to have a transplant, you will be unable to claim for any cancellation charges. ) If your treatment is completed, or if you receive the results of tests after we have issued your policy but before your trip starts, you should call the Medical Screening Line  as shown in the paragraph A above.

C Important Conditions. This insurance will not cover you in the following circumstances. At the time of taking out the policy if you -

1 have a terminal illness

2 have not been permanently living in the UK for at least the last 12 months

3 are 70 or over and buying a Worldwide Annual Multi-Trip Policy

3 are 55 or over and taking a European Annual Multi Trip Policy

At any time if you -

1 are traveling to get medical treatment abroad

2 are expecting to give birth before you arrive home or within eight weeks of arriving home

3 are not registered with a UK doctor

4 are taking a one way trip

Excess - You will pay an excess under most sections of this policy. If you claim under mote than one section, you will pay an excess for every section you make a claim for. Excess applies to every claim for every incident for each person named on the validation slip. Policy excess is up to a maximum of £75 per insured ( for some claims it will be less than this.) For anyone aged 65 plus an excess of £100 applies if you are claiming for medical and other expenses. Excess Waiver - If you have purchased excess waiver, you do not have to pay any excess. The excess waiver can only be purchased at the same time the policy is issued, and is not available for European Annual Multi Trip or Standard Cover. Even if you have bought excess waiver you will still have to pay any increased excess imposed by the Medical Screening Line due to a pre-existing medical condition or related matter.   

NB if you have to cancel or curtail ( cut short ) your trip because of something you knew about but did not tell us when you took out your policy ( eg illness of a close relative ) you may not be covered if you make a claim because of this. If you need to call the  Medical Helpline you should do so immediately. You have a 14 day return of premium guarantee. ( Full Refund less an  admin. charge of £2 ) 

Other Insurance Trap! - it is recommended that everyone travelling together insures together on the same policy especially if travelling as a family group. When one person opts for specialist cover and the rest of the party insure buys other insurance, all claims could be refused under the other insurance if there is a claim connected with one person insuring separately, eg if the trip has to be cancelled, curtailed or if there is a problem during your trip with regard to the medical condition of the person insuring separately. Travelling together? Insure together! Fortis Insurance offer a reliable 24hr Worldwide Assistance Service.  You must keep to the policy conditions enclosed with your certificate to have the full protection of your policy. If you do not Fortis Insurance  may cancel your policy or the Claims Service could  refuse to deal with your claim, or reduce the amount of any payment for a claim. 

PREMIER COVER ( Single Trip ) - NB all ages are as on date of issue of your policy. Policy valid if taken out before 30th November 2005 for travel commencing on or before 31 October 2006. Maximum trip length 185 days. Include date of return landing in UK in number of days cover required.

Important - Ages are as on the issue date of your policy or Start Date of your policy if an Annual Multi Trip Policy.
